These regimens avoids the use of cephalosporins which are a major risk for Clostridium difficile diarrhoea in hospitalised elderly patients
Levofloxacin has good activity against atypical pathogens. Addition of macrolides is not usually required.
IV Levofloxacin is not available. Tablets disperse in water after 10 mins if swallowing is difficult. IV ciprofloxacin is expensive and is a reserved drug on the hospital formulary. If IV therapy is necessary, contact a medical Microbiologist for advice.
As Levofloxacin and ciprofloxacin can increase selection for C.difficile infection and MRSA colonisation/infection, all patients with previous MRSA/C.difficile require microbiology approval to use Levofloxacin.
